Position Overview:

As a Clinical Case Manager, you will play an integral part in the care of clients. Fielding referrals and seeing them through to graduation. As a clinical case manager, you will gain experience working with a diverse population with a variety of diagnoses.

Managing Cases
Answering Phone Calls
Intake appointments
Managing Paperwork for Therapist
Oversee assessment of clients
Assist in development of client objectives.
Ensure client health and safety issues are addressed at job site.
Set up and monitor client files.
Communicate effectively with clinicians.
Mandated Abuse Reporter
Maintaining clinical records and completing documentation in a timely manner in keeping with professional standards of care
Superior interpersonal communication skills
Ensure professional writing in documentation of care provided
Maintains a high degree of knowledge and understanding of therapy practice
Possesses and conveys a high degree of both credibility and integrity
Works toward team cohesion, reinforcing to colleagues, has a positive attitude and a strong desire to make a constructive impact on organizational excellence
Maintains both the adaptability and flexibility necessary for the unpredictable nature of a busy work environment.
Administer weekly audits
Provide clinicians with resources needed to target client needs including but not limited to anger management, parenting classes, domestic violence classes, substance abuse classes, foodbanks, crisis shelters, etc.
Providing clinician support, resources, letters, per client need
Attend weekly case manager meetings
Attend specialized consultation groups on a weekly basis, take notes and follow up based on therapist request
Ability to multitask in high demand situations
