Wound care provider

We are seeking a dedicated and experienced Wound Care Provider to join our team. In this role, you will be responsible for assessing, planning, and implementing wound care treatments for patients in their homes. You will work closely with a multidisciplinary team to ensure optimal wound healing and patient outcomes.

Responsibilities:

• Perform wound assessments and develop individualized care plans.
• Cleanse, dress, and monitor various types of wounds.
• Educate patients and caregivers on wound care best practices.
• Collaborate with other healthcare professionals to coordinate care.
• Document patient progress and update care plans as needed.

Qualifications:

• Required:
  • Valid Advance practice nursing license (APRN)./ Physician assistant License
  • Minimum of 2 years of experience in wound care.
  • Certification in Wound, Ostomy, and Continence Nursing (WOCN) preferred.
  • Strong clinical skills and attention to detail.
  • Ability to work independently and as part of a team.
• Preferred:
  • Experience with home health or hospice care.
