Position Summary:
Responsible for ensuring that the residents' medically related emotional and social needs are met and maintained in accordance with Federal and State regulations and Kendal at Lexington standards in the Borden Nursing Center and Webster Assisted Living.
Job Functions:
Ensures comprehensive assessments of residents are completed.
Ensures residents and families are oriented to the Borden Nursing Center and Kendal at Lexington.
Coordinates and attends care plan meetings.
Completes mandated documentation in a quality and timely manner.
Mediates issues.
Provides links to the appropriate community resources and makes appropriate referrals.
Develop comprehensive discharge plans.
Coordinates/educates on the Resident Bill of Rights and Abuse regulations.
Participates in policy development & management.
Functions as a team member of the interdisciplinary healthcare team.
Ensures provisions of trauma-informed care.
Other duties assigned.
