Provider Care Servicer

We're seeking a compassionate professional to help make patients' transition home go smoothly. The successful candidate will work with clients to ensure they are comfortable at home, provide monitoring and help with daily household duties and errands.

Responsibilities:

• Communicate with Administrator's and Clients to maintain appropriate care is always available.
• Work with clients and families in home settings. Assisting in bathing, dressing, cooking, and light household chores.
• Accurately record patient's vital life signs (blood pressure, temperature, pulse, respirations); if needed.
• Medication reminding; assist with mobility, and shopping, and escorting patient to and from doctors appointments ( on some occasions)
• Accurately record patient's vital life signs (blood pressure, temperature, pulse, respiration rate); if needed.
• Monitor client for any additional support that may be needed

Job Type: Full-time

Salary: $9.00 - $11.00 per hour

Expected hours: 20 – 40 per week
