Clinical Manager

We are seeking an RN Clinical Manager to join our team! Looking for an energetic RN candidate to assist with start-up responsibilities.  Privately owned, Grow with us from the ground up.

Responsibilities:

• Administer nursing care to Hospice patients
• Regulatory and State Compliance
• Evaluate, admit and establish individualized patient plan of care
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians and Hospice Medical Director regarding patient care
• Educate patients and family about health maintenance and disease progression
• Facilitate referrals from other healthcare professionals and medical facilities
• Maintain accurate patient medical records, ensure regulatory compliance
• Provide advice and emotional support to patients and their family members
• On call coverage

Qualifications:

• Previous experience in Hospice preferred
• Familiarity with medical software and equipment
• Ability to build rapport with patients, community and staff
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment
