Case Manager

This position requires the candidate to be licensed as a registered nurse in Wisconsin pursuant to S.441.06, state. They must have a valid drivers license and reliable vehicle as the candidate will travel to consumers homes within our 5 county service area, for evaluations for the need for service, develop a plan of care for the client, secure written physician orders, monitor visits, assist in trainings and participate in on-call rotation.

This position would be full-time, as well as the opportunity to do some work from home. Primarily day time hours.
