Case Managers serve clients with Severe Disabling Mental Illness (SDMI) and coordinate benefits such as housing and social security. They complete an assessment of client needs and assist with resource referral and treatment planning. Case Managers work with the entire support team to coordinate comprehensive care for clients. Case Managers must follow HIPAA, maintain confidentiality, and meet productivity requirements. A bachelor's degree, relevant experience and valid driver's license is required for this position.
