Position Title: Bilingual Whole Family Approach- Case Manager/Coach
Reports To: Program Manager
FLSA Classification: Exempt
POSITION SUMMARY
The Case Manager/Coach ensures access and equitable services for families. Coaches are the bridge that connects
low-income individuals and families with critical services to achieve economic mobility.
PRIMARY RESPONSIBILITIES:
▪ Demonstrate the ability to tailor solutions to meet the psychosocial, physical, cultural, and language
needs of families.
▪ Enrolls eligible families in programs and/or services and monitors and supports program retention and
completion.
▪ Administers intakes and produces a comprehensive assessment of participants’ social emotional,
personal, educational and employment skills and goals, and support service needs.
▪ Creates and coordinates a mutually agreed-upon individual service strategy plans for each family.
▪ Meets regularly with individual families to monitor needs and progress on an ongoing basis, provide case
management, referrals, mentoring, and coaching.
▪ Records families’ interactions, services, and events in a timely and proficient manner, to ensure that goals
and objectives are met. Maintains and updates participant plans and files in accordance with program
standards and contract requirements.
▪ Maintains organized and updated physical and electronic files on each family, including relevant
documentation, signed forms and releases. Implements evaluation aspects of all program components
ensuring confidentiality of data related to intakes, follow-up, exit, and satisfaction surveys. Adheres to all
federal, state, and local document confidentiality requirements.
▪ Supports Identity's mission and values. Continually look for ways to improve the organization. and works in
alignment with Identity's strategic goals.
SECONDARY RESPONSIBILITIES:
▪ Attends and actively participates in required training, meetings, and presentations.
▪ Participates in organization's outreach and advocacy activities.
▪ Coordinates with other agencies and partner programs.
▪ Performs other duties as assigned.
EDUCATION, EXPERIENCE, and REQUIREMENTS:
▪ Bachelor’s Degree preferably in Social Work, Sociology, Psychology, or related field, or 5 at least years’
experience as a case manager or related position.
▪ Knowledge of community resources
▪ Experience using database software desirable.
▪ Must be able to pass a background check and a Child Protective Services
background check
