Care Management

Job description

Our Care Managers provide education, connect resources, advocate and coordinate matters of aging for our private client families. 
- Conduct patient assessments and develop individualized care plans 
- Coordinate patient services and ensure proper implementation of care plans 
- Collaborate with healthcare professionals to optimize patient outcomes 
- Monitor and evaluate patient progress and adjust care plans as needed 
- Provide education to patients and families on healthcare management 
- Maintain accurate and up-to-date patient records 
- Adhere to HIPAA regulations and infectious disease care protocols 
*Qualifications* - Background or experience in senior care preferred
 - Excellent communication and interpersonal abilities - Ability to work effectively in a fast-paced environment 

Job Types: Part-time, Contract, PRN

Pay: Probationary period $21.50 per hour with potential to increase following 90 day probationary period

Expected hours: 5 – 20 per week

Location: Wake Forest/Youngsville/Rolesville 

Work Location: Remote
