Are you a compassionate RN with hospice experience looking to make a real difference in patients' lives? We're hiring a Hospice RN Case Manager to join a mission-driven team dedicated to delivering exceptional end-of-life care in the San Diego community.

Position Overview

As a Hospice RN Case Manager, you will lead the coordination and delivery of patient-centered hospice care. You’ll support patients and their families with empathy, clinical excellence, and professionalism, while collaborating with an interdisciplinary team to ensure the highest standard of care.

Key Responsibilities

Plan and manage individualized hospice care plans based on patients’ physical, emotional, and psychosocial needs.

Conduct patient assessments and adapt care plans in collaboration with the Clinical Manager.

Supervise and guide hospice aides, ensuring compassionate and consistent care delivery.

Educate families on end-of-life options and provide supportive communication throughout care transitions.

Partner with interdisciplinary team members (physicians, social workers, aides) to deliver holistic care.

Minimum Requirements

Candidates must meet all requirements to be considered:

Graduate of an accredited nursing school with current RN licensure in California.

1–2 years of recent acute care experience in a clinical setting.

1–2 years of home health or hospice care experience (preferred but not required).

BLS Hands-On CPR certification (active).

Strong communication, observation, and decision-making skills.

Valid driver’s license, reliable transportation, and auto insurance (required for home visits).
