Case Manager (COAST)
Description:
United Services has an exciting opportunity to be at the forefront of a new program, COAST (Community Outreach Assistance Support Team), and seeks passionate, dedicated case managers to work with individuals who need multiple touchpoints during the week to stay stable in the community. The right candidate will also coordinate care with providers, working with individuals on caseload so all collaborate to meet individuals where they are at so their recovery and wellness are enhanced.
As a case manager you have a unique opportunity to forge a therapeutic connection with an adult who has been diagnosed with a mental health disorder. When individuals are asked what made the difference in their lives, the most frequent response is that they had someone they worked with who gave them hope that they can have what everyone wants: safe and decent housing, access to quality mental health and medical care, meaningful personal relationships, productive activities including work, and a connection to the community in which they live.
Case Manager (COAST) Schedule:
Monday – Friday 8:30 am – 5:00 pm with flexibility and possible weekend rotation.
Case Manager (COAST) Minimum Requirements:
Associate’s Degree in relevant human services field plus two years of relevant experience or Bachelor’s Degree in relevant human services field.
Salary Range:
Salary/Hourly Range: 18.54 – $20.98 per hour depending on experience
Bilingual Spanish/English Fluency: $19.48 – $22.04 per hour depending on experience
