Case Manager for Veterans Housing Program

The Veterans Accession House (VAH) is a Nonprofit organization contracted through the Department of Veterans Affairs to provide transitional housing for male Veterans in Contra Costa County.  We are seeking qualified applicants for the position of Case Manager. The CM works directly with program participants in Service Intensive Transitional (SITH) and Bridge Housing program setting to provide services consistent with the program and the needs of individual clients. Case Manager possesses the education, training, and working knowledge of practices and strategies needed to conduct weekly Case Management sessions. Session focus on design of client centered Service Plans to prepare them for independent living. The CM models a positive outlook and a passion for effective fieldwork habits to motivate and encourage veterans to take necessary steps to overcome barriers to attaining and or retaining housing.   

Summary

The case Manager provides supportive services for residents and assists them in resolving personal issues and making decisions as necessary, to effect positive outcomes. CM works collaboratively with the program Case Manager Supervisor, Field Service Manager, and House Managers to achieve program personal growth and effect change. Responsible for coordinating case management and program services for an assigned caseload of male Veteran residents, implementing and adheres to the Critical Time Intervention case management model. Assist Veterans to establish a network of supportive services and benefits. Assist each Veteran to move toward self-sufficiency and readiness for independent living. Able to apply crisis intervention strategies and coordinate behavioral health services with local care providers. Promote a positive, professional attitude towards Veterans, staff members, and other collaborative partners.
 

Essential Duties and Responsibilities
Provide case management and intervention strategies to program residents. Assesses Veterans to determine readiness and appropriate housing placement. Conduct vocational, services, and benefits-related needs assessment. Assesses clients for strengths, and barriers, including employment, training, and behavioral health needs, etc., and refers the participant to appropriate services. Assists in the development and monitor realistic service/housing stability plans. Identify specific actions needed to stabilize and create a successful program experience. Completes all Charting and Documentation requirements, including Intake Documents, case management case notes, Service Plan, quarterly progress notes, quarterly progress notes, and Discharge Plan within the specified timeframes. Educates participants about budgeting and financial management, including support with banking and credit repair. Refers to veterans to appropriate public benefits and services to stabilize and improve financial standing and increase housing options. Assists with collecting documentation and provides transportation to housing, employment, and medical appointments as needed. Schedules and maintains weekly meetings with Veterans. This may require the flexibility of meeting locations between the VAH office, Veterans assigned housing unit, and remotely. 

Administrative

  • Ensure that Veterans complete program exit surveys upon discharge.  
  • Gather statistical data to monitor program effectiveness.
  • Maintains an accurate account of Veteran progress, via case file progress notes.
  • Maintain the case management contact log at all face-to-face client visits, e.g., housing counseling, follow-up, referrals, and non-face-to-face client management.
  • Ensure that all client contacts are recorded in the client’s progress note.
  • Ensure that copies of all referrals to clients are included in each client case file, including referrals to benefits, health care, housing, and community. 
  • Coordinate with local Veterans Affairs HUD-VASH Program case managers to discuss VAH program residents’ readiness and eligibility for housing placement. 

Qualifications: Requires excellent oral and written communication skills, and good organizational and interpersonal skills. Knowledgeable of or capable of learning VA resources, community resources, and service providers who work with veterans. Self-directed, yet willing to request guidance or assistance when needed. Thorough, well-organized, and competent in managing detailed case files.

EDUCATION and EXPERIENCE:  A minimum of a Bachelor of Science degree is required, or a master’s degree is desired. Case management experience caring for the target population desired.

CERTIFICATES, LICENSES, REGISTRATIONS: CA Driver’s license and insurance

Employment Type

  • Less than Full-time 24 – 32 hours per week.
  • Hours of work are to be performed Monday through Friday by arrangement. 
  • Salary $25.00 - $30.00 per hour
