We are seeking a Physical Therapist to join our team! This role is responsible for conducting initial evaluations and accurately assess patient conditions to develop appropriate goals and treatment plans through education, coaching and graduation (discharge) planning for patients.

Duties & Responsibilities:    

  • Evaluates physical and functional status, as well as cognitive, psychological, and perceptual ability to determine the patient’s capabilities to function (safely)
  • Documents all evaluations, treatment, and coordination of care contacts via appropriate facility policies
  • Demonstrates an ability to treat a wide range of diagnoses and be knowledgeable in current therapy treatments, goals, and contraindications for treatment
  • Provides education and counseling to patient/family regarding treatments and goals, potential safety risks and/or other issues within scope of practice
  • Independently manages own case load to maximize effectiveness of overall care plan
  • Complies with all applicable Federal, State, and local laws, regulations, and requirements, as well as facility/agency specific policies and procedures.   
  • Supports special projects and other designated activities as assigned in support of TAI’s mission and core values  

Minimum Qualifications:  

  • Bachelor’s or higher degree in Physical Therapy from an institution accredited by Commission on Accreditation in Physical Therapy Education
  • Current State Physical Therapist License
  • Current CPR certification
