Responsibilities:

Family Communication:

Engage with families to understand their challenges and needs.

  • Client Assessment: Evaluate potential clients to determine if they are a good fit for our programs.
  • Intake Board Review: Analyze and filter the client list based on program suitability.
  • Documentation Management: Gather and ensure the completeness of all necessary documentation from clients.
  • Client Submission: Submit clients to the appropriate Case Management Agencies (CMAs). • Coordination: Maintain regular contact with providers and administrative staff at the clinic to ensure proper documentation and follow-up.
  • Client Progression: Ensure clients are moved forward in a timely manner through the intake process.

Pay: $25.00 - $30.00 per hour
