Ingleside Communities has a unique full-time opportunity for a long-term care Social Worker.

As the Social Worker, you will work under the direction of the Nursing Home Administrator and be responsible for promoting the general emotional and psychosocial needs of residents. You will act as the liaison among departments within Ingleside Communities as well as the discharge planner working with community resources to ensure resident needs are met after they are discharged. You will complete resident interviews and assessments, coordinate resident care plan meetings and develop and execute resident care plans based upon resident needs following state and federal regulations.

Requirements

  • Bachelor's degree in Social Work or Human Services (sociology, Rehab Counseling, Psychology)
  • Minimum one-year experience as a Social Worker in a long-term care setting (preferred)
  • Knowledge of and ability to obtain community resources
  • Above average organizational skills
  • Demonstrated excellent oral and written communication skills.
  • Fluent in the English language with ability to read, write and speak English
  • Commitment to a multi-disciplinary approach to resident care
  • Demonstrated understanding and adherence to state and federal regulations
  • Flexibility with schedule to meet resident needs

Essential functions:

  • Demonstrates working knowledge and ability to apply professional standards of social work practice in job situations.
  • Demonstrates in-depth knowledge of state and federal regulations that apply to practice of social work in long-term care. Keeps abreast of changes with state and federal regulations and makes recommendations on changes in policy/procedures to administration.
  • Develops and maintains customer friendly service skills with all departments within all departments within Ingleside Communities.
  • Ensures social service programs are properly maintained to meet the needs of the resident.
  • Maintains and reinforces confidentiality of all resident care information and ensures resident rights are protected.
  • Provides yearly in-service to employees regarding resident rights and confidentiality.
  • Collects data from resident and family interviews in a timely basis to assess the psychosocial factors associated with resident's illness/disability. This data will be used for MDS, billing purposes and goal setting with the care team.
  • Develops and maintains departmental policies, procedures, forms, goals and objectives.
  • Works with the Financial Manager to provide the resident and family assistance in understanding financial programs available through Medicaid/Medicare and other programs available.
  • Coordinates the resident care plan meetings, sends letters and keeps resident/family informed, coordinates goals for each resident with multi-disciplinary team.
  • Provides consultation to members of staff, community agencies, etc. to work toward solutions for needs and concerns in the best interest of residents and their families.
  • Provides educational opportunities for residents, families and the community at large including monthly support group meetings.
  • Develops and maintains working relationships with community health, welfare and social agencies and seeks creative means to assist the patient with their needs.
  • Plays an active role in the review of complaints and grievances and assists the Administrator and Director of Nursing with compliance in state and federal regulations.
