JOB PURPOSE: The role of a Dietitian at AHN is to oversee the nutritional well-being of our clients by providing medical nutrition therapy and health education services. The successful candidate will have a proven track record of conducting nutrition assessments and creating appropriate nutrition plans accordingly. You should be able to analyze the effectiveness of interventions and use metrics to implement data-driven improvements. You should also actively keep up with the latest nutrition research, integrating new findings into diet plans and nutritional services. The ideal applicant has experience providing nutrition education to patients and mitigating risk factors. You must be ready to collaborate with all members of a client's care team including physicians, nurses, care managers, and care givers to tailor dietary services and improve patient outcomes. The Registered Dietitian should be a great communicator, critical thinker, and leader.
BASIC REQUIREMENTS:
- Bilingual (English/Spanish) preferred.
- Required to pass a background check through the State of Nevada.
- Must have a valid driver's license and be able to drive the AHN company car when needed.
- Process, maintain and gather protected health information from clients and program partners in a manner compliant with all HIPAA regulations.
- Abide by all AHN policies and guidelines.
KEY RESPONSIBILITIES AND ACCOUNTABILITIES:
- Assess client's needs and works with the team to develop an individualized education plan of care to facilitate the development and application of needed skills to positively impact disease management.
- Identify patient's specific disease self-management and medical nutrition therapy goals and develop unique educational care plan to best meet individual needs and lifestyle.
- Implement personalized, age-specific and culturally appropriate nutrition strategies.
- Confer with referring provider or provider's office staff regarding patient's individual care plan and progress; establishes and records baseline data for use in monitoring and evaluating ongoing progress.
- Facilitate health education with patients and families in disease self-management, identify self-management objectives, and modify behavior/lifestyle; topics include; overall disease management, coping with the disease and managing risk factors; motivational techniques, healthy eating and physical activity; how their medications work, monitoring health status; explaining the risk of complications; documents education activities and interventions in accordance with established procedures.
- Conduct individual counseling sessions for clients who need to work on specific disease self-care skills to reduce existing risk factors or for other medically necessary reasons.
- Create and maintain positive working relationships with AHN program partners based upon a foundation of diligence, timeliness and follow through.
- Work with the client and care team to identify clinical, socioeconomic and mental/emotional barriers to them achieving positive health outcomes.
- Screen for and refer to appropriate health-related programs and classes.
- Act as an advocate for the client as needed/when appropriate with program partners, treating physicians/providers, insurance companies, community organizations to ensure the client is receiving the care and resources they need.
- Participate in team meetings designed to enhance the effectiveness of both individual staff and the program as a whole.
- Make sure that all patient records are up to date with the necessary documents and screening tools.
- Maintain proper notes and documentation through accurate and efficient recording of all pertinent information into the database system.
- Fulfill relevant parts of the Scope of Work for funding grant.
- Ability to manage an assigned caseload of a diverse population including individuals living with HIV/AIDS
- Respectfully interact with clients while working cooperatively in a team environment.
- Ability to be both compassionate and professional while serving our population.
- Ability to successfully engage and communicate with patients/clients to achieve desired behavior modification and associated health outcomes.
- Must successfully complete required training in topics such as: motivational interviewing, chronic disease self-management, diabetes self-management, effective communication, health coaching, etc.
- Must be willing and able to see clients in a hospital and/or clinic setting if necessary.
- Flexibility: each department selects the hours of work schedule for the department staff based on the department's needs. This schedule may change at any time according to the department's needs.

EDUCATION MINIMUM:
- Bachelor's degree in related field from an accredited college or university.
- Must be a current Registered Dietitian.
EXPERIENCE MINIMUM:
- One+ years of professional experience as a registered dietician - preferred.
- Experience in providing medical nutrition therapy to at risk populations.
- Experience in providing disease self-management education to at risk populations.
- Experience in working with socioeconomically disadvantaged populations.
- Experience working with databases and software programs such as: Microsoft Word, Excel and Power Point.
- Excellent interpersonal skills.
- Excellent customer service skills.
WORK ENVIRONMENT:
- The noise level in the work environment is usually moderate and the employee usually works in a climate-controlled office environment.
- Be able to stand between 4 -6 hours per day.
- Be able to sit at a computer for up to 4 - 6 hours per day.
- Be able to withstand hand/wrist deviation and repetition.
- Must be able to lift, carry up to 30 pounds with a fair consistency.
