Schedule: Mon-Fri, 8:00AM-5PM (1 hour lunch)-Flexible for 4, 10 hour shifts

JOB RESPONSIBILITIES:
-- Consult with patients to learn of their physical condition symptoms and medical history and referrals from care teams
-- Diagnose any movement dysfunctions or impairments
-- Develop a specific/individualized treatment plan to include objective measurable assessments
-- Coaching and teaching patients therapy exercise to improve their condition
-- Maintain patient records which include tracking of goals, progress and other pertinent information
-- Communicate effectively with all members of care provider team including patient family/guardian(s) per HIPAA guidelines,
-- Communicate with the DOR tasks such as educational needs for team are met-which could include assisting with In-Services, peer reviews, ensuring license requirements are current etc.

EXPERIENCE AND QUALIFICATIONS:
-- Current Physical Therapy License applicable to Washington DC
-- Physical Therapy Degree
-- Basic computer literacy skills
-- CPR certified
-- Medicare billing experience, utilizing CPT codes, documentation and guidelines
-- Organized patient care documentation and facility documentation skills
-- Geriatric experience-not limited to either LTC or home or another environment
-- Experience working with ventilator and trach patients preferred
-- Professional demeanor, dress code adherence, and overall, your best presentation is a must

CONDITIONS & PHYSICAL REQUIREMENTS
-- Must be able to safely work with patient transfers following all guidelines which may include patient specific instructions from care team and / or facility
-- Must be able to work individually and colleagues/team to provide safe environment for all - yourself and the patient
-- Environments are SNF and LTACH setting(s) so the ability to walk, assist and be mobile is a must (bending, stooping, crouching etc)

Job Type: Full-time

Pay: $90,000.00 - $95,000.00 per year
