Job Description

We are seeking a motivated and dedicated Per Diem Behavioral Health Case Manager to join our dynamic team. This position offers a flexible and fun environment that supports professional growth and upward movement within the organization.

Responsibilities:

• Conduct comprehensive assessments to identify client needs and strengths
• Develop and implement individualized service plans (ISPs)
• Provide case management services including coordination of care, referrals, and follow-up
• Facilitate group and individual counseling sessions
• Monitor and evaluate client progress, adjusting treatment plans as necessary
• Maintain accurate and timely documentation of services provided
• Collaborate with multidisciplinary team members to ensure holistic care
• Participate in training and professional development opportunities

Qualifications:

• Bachelor’s degree in social work, psychology, or a related field 
• Minimum of 2 years of experience in case management or behavioral health services (preferred, but not required)
• Licensed in the state of Florida (Certified Behavioral Health Case Manager) preferred
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Proficiency in electronic health records (EHR) systems
• Commitment to providing high-quality, client-centered care
