We are seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will be responsible for providing support and assistance to clients in various aspects of their lives throughout the full client care cycle, including but not limited to obtaining identification documents, applying for benefits and accessing medical, mental health, and substance abuse services. The Case Manager will also be responsible for completing intake paperwork, submitting case notes, completing/submitting monthly goal plans, completing/submitting monthly reports, and creating and maintaining client files. Additionally, the Case Manager will work with clients to create budgets, assist them in finding permanent housing options, and meet with each client on their case load on a weekly basis. The Case Manager will also be required to attend weekly clinical meetings to review and discuss each client’s case progress.

Key Responsibilities:

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
  • Help clients make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Develop effective working relations and cooperate with medical team throughout the entire case management process
  • Record case information, accurately complete all necessary forms and produce statistical reports
  • Adhere to professional standards as outlined by protocols, rules and regulations
  • Handle case assignments, individual plan of cares, document/review case progress and determine case closure
  • Assist clients with obtaining identification documents as needed
  • Assist clients with applying for benefits as needed
  • Assist clients with setting appointments with medical, mental health, and substance abuse providers
  • Complete intake paperwork with clients
  • Complete and submit monthly goal plans
  • Complete and submit monthly reports
  • Create and maintain client files
  • Create budgets with clients that have secured income
  • Assist clients with finding permanent housing options when they’re ready to transition to independent living
  • Meet with each client on case load on a weekly basis
  • Attend weekly clinical meetings to discuss each client's progress

Job Type: Full-time

 

Pay: $20.00 per hour
