EverMind is a behavioral health agency that seeks to provide comprehensive behavioral health care to people living in assisted living, skilled nursing and the group home environment. By partnering with long term care facilities, we improve collaboration within the care team, provide more accurate diagnosis, and provide the most evidence based mental health treatments.

Through our relationship with each facility, we hope to prolong independence, improve quality of life, and reduce caregiver fatigue in both nursing staff and family members.

Services are provided to the client’s within their living facilities. Expected hours are Monday- Thursday 9-6 and Fridays 9-1 pm.

 

What we are looking for:

EverMind is seeking a CBRS/Case Manager to join our clinical team. This CBRS/Case Manager will provide mentoring to clients in assisted living. The ideal candidate will have experience working with geriatric, TBI and/or SMPI clients. This CBRS/Case Manager should also be able to work independently.

 

Responsibilities:

● Ability to complete assessments, treatment plan reviews, progress notes, and discharge summaries

● Organized and ensures all documentation is completed within a timely manner

● Able to adhere to payor and company standards

● Attends regularly scheduled supervisions/staff meetings

● Collaborates care with other treatment providers

● Complies with HIPAA privacy & practices

● Performs other duties assigned or necessary to support the program and/or company

● Assess emergency situations, notify supervisor and/or appropriate personnel of actual and potential issues

● Maintain a nonjudgmental approach, effective listening, eye contact, professionalism, and positive interactions

● Provide consistency and support to promote personal progress and accountability

Special Conditions:

This is an Independent Contract position and compensation will be paid per billable code.

 

Qualifications

● Bachelor’s degree required, Social Work is a plus

● CPR Certification is a plus

● Ability to pass enhanced IDHW background check

● Valid driver’s license with reliable transportation and proof of car insurance

● Ability to work in a community-based setting

● Experience utilizing electronic health records is a plus

 

Requirements:

● Cleared enhanced background check, performed by IDHW

● Proficiency and use of Electronic Medical Records System with appropriate training

● Maintain HIPAA compliance

● Attend all staff meetings, staff developments, and staff training

Ability to work independently, but with the support of a team and strong supervision.

Salary: From $19 to $21.00 per billable hour
