Full job description

We are seeking a Licensed Clinical Social Worker to join our team to provide holistic substance abuse treatment to clients through individual, group, and/or family counseling sessions and develop case plans while working with interdisciplinary treatment team to provide continuity of care to client. These duties apply to P&N Behavioral Health, LLC.

GENERAL RESPONSIBILITIES:

1. Complete Biopsychosocials to evaluate client health and substance use issues as needed

2. Complete Assessments

3. Diagnose substance abuse and mental health symptoms accurately

4. Identify issues and create treatment plans in collaboration with client and according to state of Kentucky regulations

5. Orient clients to program operations and procedures

6. Conduct individual, group, and family counseling sessions according to program guidelines and state of Kentucky regulations

7. Maintain client records within EHR that are complete, comprehensive, and well-organized

8. Make appropriate referrals to other services as needed

9. Fill in to provide intake services, including several weeks of supportive follow-up counseling for new clients until their assignment to a primary Clinician

10. Provide crisis interventions/support services to clients

11. Utilize evidence-based therapeutic interventions to assist clients with reaching treatment goals and re-integrating into society in a way which promotes self-sufficiency and healthy living

12. Obtain number of continuing education units as required by individual licensure board

13. Comply with all code of ethics, guidelines, and regulations as outlined in the P&N Behavioral Health Employee handbook, Licensure board requirements, and state of Kentucky regulations

14. All other duties as assigned

BACKGROUND/EDUCATION

1. An individual with a master’s degree or greater in any counseling associated field from an accredited college or university

2. An individual with appropriate credentialing and licensure, to include established supervision if not fully licensed, by a licensure board approved and recognized by the state of Kentucky

3. Demonstrate proficiency in both group and individual therapeutic modalities

4. Demonstrate ability and proficiency in developing treatment plans with specific goals and objectives corresponding to each client’s needs

5. Demonstrate ability and proficiency to meet with clients to evaluate their substance abuse and/or mental health problems

6. Demonstrate ability and proficiency in delivering appropriate therapeutic modalities and interventions

7. Ability to work in conjunction with interdisciplinary team to coordinate needed services for client

REQUIRED QUALIFICATIONS

Must have knowledge of medical privacy and confidentiality laws (Health Insurance Portability and Accountability Act [HIPAA]); computer application software to include Microsoft Office and internet familiarity

Experience working with individuals and families who have various mental, behavioral, and substance abuse conditions

Familiarity with Best Notes preferred but not required

SKILLS REQUIRED

1. Must have organizational and communication skills, both written and verbal

2. Communicate effectively in oral and written format

3. Maintain effective interpersonal relationships

4. Must have ability to work independently with minimal supervision

5. Must have ability to work in a stressful and fast-paced environment

6. Ability to analyze treatment plans, which include the diagnosis of client behavioral, mental, and emotional symptoms

7. Demonstrate knowledge of a variety of therapeutic models and theories to understand and plan intervention strategies for each client from a person-centered, strengths-based perspective

8. Monitor client progress and adjust therapeutic approach according to best practices utilizing evidence-based intervention based on client response to treatment

9. Work collaboratively with the patient and interdisciplinary team to develop a comprehensive treatment plan that addresses priority needs of the client, desired treatment outcomes, an agreed upon plan of action, and reassessment

WORKING CONDITIONS

Physical Demands

Must be able to sit, stand, bend, reach overhead and lift up to 10 pounds; travel outside of the office and/or attend meetings both during and after normal work hours, both at or away from the work site. Exerting up to 10 pounds of force occasionally or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body. May involve walking or standing for brief periods of time. Requires about 6 hours of sitting and no more than 2 hours of standing or walking in an 8-hour workday. No significant stooping is required.

Environmental Conditions

The employee will work in an indoor setting with central heat and air conditioning. The structure itself is open and ventilated well with central heat and air conditioning. The employee may work outside in heat/cold, wet/humid, and/or dry/arid conditions during times of community activities for clients.

Mental Demands

There are a number of daily deadlines associated with this position which may cause significant levels of stress to some individuals. This position requires daily interactions with clients, staff, and insurance representatives on a variety of issues.

WORK EXPECTATIONS

1. Minimum of 40 hours per week including rotating on-call scheduling

2. Must be able to work well with staff and clients

3. Minimal travel required
