We are seeking a Physical Therapist OR Occupational Therapist to join our team! You will provide direct patient care and support rehabilitation activities.

Responsibilities:

• Conduct an in person initial comprehensive occupational / physical therapy assessment on all patients
• Review the plan of care at least once every 30 days after new or revised plans of care, document in the medical record that the plan and the participant’s progress have been reviewed with the patient and other medical staff, as needed
• Re-evaluate participant’s level of function to objectively determine participant’s response to treatment. Alter treatment plan and activities accordingly.
• Facilitate rehabilitation of patients with physical injuries and/or disabilities
• Performs FCE, Work-Hardening, ROM and/or other testing as ordered by medical doctor
• Monitor and assess patient rehabilitative programs
• Convey treatment results and patient progress to relevant parties
• Organize and update patient records on a daily basis in EHR system
• Actively participates in the initial evaluation and patient status meetings.
• Maintain a clean and healthy environment

Qualifications:

• Previous experience in occupational / physical therapy or other related fields (Experience with work injuries a plus)
• Ability to build rapport with patients
• Passion to help and motivate others
• Excellent written and verbal communication skills
