Licensed Social Worker

We are currently looking for a part time licensed Social Worker. Telehealth and office-based opportunities are available.

Requirements:

To be eligible each applicant MUST have a current license in the State of Ohio as a Counselor, LSW, LPC, LPCC, Social Worker, or LISW.

Duties include:

· Providing individual, family and/or coupes/marital therapy or counseling to adults or children

· Treatment Planning

· Progress Notes Review

· Assessments

· Transition/Discharge Summaries

· Thorough documentation of screening/assessments, evaluations, notes, contacts made must be accurate and timely.

· Position is responsible for coordinating care with the Case Manager and other care providers.

Job Type: Part-time

Salary: $33-38 per hour

Schedule: Part-Time

• Day shift
• Monday to Friday
