The Plymouth Lead Agency - Program Director provides intensive care coordination, direct supports, and network linkages to children, families and the Department with the goal of developing and implementing integrated plans of care that foster increased stabilized community tenure as well a safety, well being, and permanency.
What You Will Be Doing to Make a Difference...
• Develop and maintain collaborative relationships with state agencies, residential programs, group homes, service providers, and community organizations at the area, regional, and state levels
• Develop community partnerships with formal and informal community resources thus enabling access to expanded, ongoing community-based services for the children and families of DCF
• Develop a Network and partnership of Preferred Providers who support the creation and implementation of a service delivery system that is integrated, collaborative, flexible, and responsive to the unique needs of the children and families being served
• Identify and actively support the development of new and necessary service models supporting permanency and community tenure
• Partnership and collaboration with Area DCF Administration and Regional Resource Center staff in all requested meetings and initiatives
• Maintain direct caseload as determined by the Department
• Engage families and their identified supports/services in a collaborative commitment to identify family culture, strengths, needs, and goals
• Complete individualized family culture discoveries, strength-based assessments, and safety plans
• Coordinate and facilitate Network planning meetings to support successful outcomes as defined by the family and the Department
• Collaboratively implement and monitor individualized plans of care supporting diversion from out-of-home placement and return to the community from residential/group care
• Implement performance and resource management reports to guide and monitor strategic Network development
• Comply with financial monitoring methods and reports providing information regarding utilization of available funds by amount and by service/provider
• Develop with and manage for the Area Office a system of management and reporting for Flexible Funds
• Provide clinical consultation for all DCF staff
• Respond to DCF crisis management needs and emergency access of services
• Participate in DCF Regional and Area initiatives to include but not limited to MDAT, Permanency Planning Conferences, and Group Care Reviews
• Participate in identification and resolution of issues regarding Family Networks Program with the staff of DCF, Lead Agency, Network of Providers as well as family members

Qualifications/Credentials
• Master’s degree in Human Service field
• Licensure by the Commonwealth of Massachusetts (LICSW, LMHC, LMFT, LADCI) preferred but not required.
• Demonstrated participation in multidisciplinary teams and/or collaborative systems
• Demonstrated prior experience of working with and understanding the vision, mission, and values of the Department
• Knowledge of and the ability to assess: child and adolescent treatment, family systems dynamics and treatment, strength-based/solution focused methodologies, behavioral interventions, substance abuse and domestic violence treatment, crisis management to include crisis prevention planning and direct intervention
• Minimum of 5 years of experience working in child and family treatment (assessment, treatment planning, outcome measurement, with systemic, behavioral, and developmental focus)
• Demonstrated competencies in the area of family systems, substance abuse, and domestic violence
• Broad-based knowledge of collaborative partnerships
• Systems orientation with a proven ability to think and act under the guiding principles of family centered practice
• Good organizational and strong interpersonal skills
• Ability to motivate others and manage conflict
• Capacity to work within greater Coastal or Plymouth Area and larger South Shore Area
• Valid MA driver’s license with acceptable driving record and current proof of insurance
• Acceptable CORI and SORI background record check as required by program
