Supervisor: Health Care Partnerships Program Manager

Salary Range: $50,000-$55,000

 

The Role: The Health Care Partnerships Program Associate will play an essential role in supporting the Flexible Services Program. Nutrition Services Coordinators will speak with clients referred to us by our health care partners and determine which goods and services are needed. The Program Associate will then fulfill service orders to ensure that clients receive the needed items.  Throughout the duration of the program, they will support clients with various requests and direct them to helpful resources.

 

About Flexible Services

The Commonwealth of Massachusetts received a waiver from the Centers for Medicaid and Medicare to improve the ways in which MassHealth provides care to its members. This waiver led to the establishment of a pilot Flexible Services Program.  Through the Flexible Services Program, Accountable Care Organizations (ACOs) may pay for health-related nutrition and housing supports as part of their strategy to address social determinants of health.

Project Bread is an approved nutrition service provider for three ACOs.  In this role we receive referrals and provide nutritional services to members from over 45 health centers, hospitals, and physician offices across Massachusetts.

 

Primary Responsibilities:

  • Fulfill orders for kitchen supplies.
  • Coordinate the delivery of goods and services to clients by working with their coordinators to ensure lost items or returned mail arrive in the hands of clients.
  • Provide daily assistance to individuals utilizing our food benefit card service, monitor usage, serve as the primary point of contact for customer service inquiries.
  • Answer phone calls, emails, and voicemails from clients of the Flexible Services Program.
  • Support cooking classes by scheduling, managing translation service, and sending reminders to clients.
  • Provide support for cooking and nutrition education classes.
  • Assist with record keeping and reporting as needed.
  • Perform other related tasks as needed.

 

Spanish-Language Support Responsibilities:

  • Regular phone conversations to provide technical assistance.
  • Interpret cooking classes.
  • Review translated materials for cultural and contextual relevance.

 

Organizational Responsibilities

  • Participate in planning and evaluating the annual Walk for Hunger, including working as event staff the weekend of the Walk (first weekend in May), and attending pre-Walk events as assigned. 
  • Participate in organization-wide events and meetings. 
  • Act to promote, sustain, and grow Project Bread’s positive, productive, and inclusive working environment. 

 

Qualifications:

  • Strong organizational skills.
  • Ability to manage multiple tasks simultaneously.
  • Database experience (Salesforce preferred).
  • Strong written and oral communication skills.
  • Experience working in diverse environments with people of different backgrounds.
  • Fluency in Spanish.
  • Ability to work one evening a week.
  • Prior professional experience, preferably in customer service or public service.

 

All employees are currently working a hybrid remote and in-office model and are expected to be present in the East Boston office on a regular basis. Remote work schedules are determined by departments. The Program Associate will work from the office two (2) days a week.  This position will require evening availability one day a week.
