ProMedica Hospice, formerly Heartland Hospice, is currently hiring a full-time Medical Social Worker to support hospice patients and families in their homes.  This is a full-time M-F day shift opportunity with a limited after hours on call rotation.  

The Medical Social Worker is responsible for providing patients and families psychosocial care regarding economic, social, and emotional needs.

Requirements

Prefer one year experience in a health care setting. 100% travel necessary on a daily basis. Demonstrate problem solving and decision making skills. Demonstrated ability to work will with an interdisciplinary team. Excellent communication skills. Ability to develop and maintain rapport with the client and family.

Educational Requirements

Masters Degree in Social Work from an accredited institution. May require licensure or certificate as required by the state, and in good standing. (if applicable)
