Responsibilities:

• Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists.
• Review patients’ functional needs and adjust care plans to ensure optimal patient outcomes.
• Design and implement a plan of care for patients based on a thorough assessment of potential development and incorporate participation by significant others whenever it seems appropriate.

Requirements:

• NH licensed PT

Salary: Up to $104,000.00 per year
