Targeted Case Manager
EDUCATION:
Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development).
EXPERIENCE:
One (1) year minimum of full time or equivalent experience working with children and/or adults with serious emotional disturbances.
LICENSURE/CERTIFICATION:
Has completed or agrees to complete AHCA-approved mental health targeted case management training within three (3) months of initially providing Medicaid services.
COMMUNITY SERVED: Children and Adults
KNOWLEDGE AND TRAINING REQUIRED:
Have knowledge of available resources in the service area for children with serious emotional disturbances
Is knowledgeable of and comply with state and federal statues, rules, and policies that affect the target population
Effective time management skills
Ability to maintain confidentiality
Good communication skills
Understanding of cultural diversity
Ability to travel
Effective problem solving skills
Accurate Assessment Skills
Ability to use computer/tablet/PC, calculator, telephone system, fax/copy machine, and other needed machinery/electronics to accurately provide services to service population.
Ability to perform telemedicine services when necessitated.
Job Type: Contract
Salary: $18.00 - $18.50 per hour
