Position: Visiting Nurse Practitioner

Employment type:

Full-time - 36-40hrs

Part-time

Salary:

$93,000-$150,000

Job Type:

In clinic, mobile, facility/home.

Work vehicle provided.

Shift and Schedule:

Monday- Friday

8-hour shift

No weekends.

No on call.

FNP position, hours and salary will be determined at time of hire.

The Nurse Practitioner (NP) will utilize the nursing process to care and/or counsel individuals in the promotion and maintenance of health.

The NP may perform the following:

the identification, assessment and delivery of patient care
the creation of effective teaching materials and beneficial health promotion activities for patients consistent with Agency philosophy, policy, objectives and Standards of Clinical Practice
the review of documentation to ensure accuracy and compliance with regulation
the coordination of day-to-day activities
entering, collecting, researching, compiling and/or verifying data
identifying problems and resolving/escalating issues as necessary.
The Nurse Practitioner will be assigned to a licensed Nurse Practitioner preceptor during the probationary period and/or during the credentialling process.

Nurse Practitioner (NP) will work closely with patients in all settings to support patients in achieving their healthcare goals. Primary responsibilities includes supporting treatment of patients as assigned, contributing to treatment goals alongside multidisciplinary clinical team, diagnosis, treatment, coordination of care, preventive care, medication management and oversight.

Decisions are limited to defined parameters around work expectations, quality standards, priorities and timing, and works under close supervision and/or within established policies/practices and guidelines with minimal opportunity for deviation.

Required Qualifications:

1 year of experience as an advanced practice provider

A clinical background in adult, family, or geriatric specialties

Graduate of an accredited Nurse Practitioner program

Board Certified/Board Prepared through AANP, ANCC or NCCPA

Nurse Practitioner national certification as ANP, FNP, or GNP required or the ability to obtain.
Current, unrestricted NP license in the state of assignment or the ability to obtain license.
Ability to obtain and/or maintain State DEA licensure / Prescriptive Authority post - where applicable.
Preferred Qualifications:

· Bilingual

· 1 year + experience in visiting provider service as FNP

· Advanced phlebotomy

· Wound care

· Mental health

Licensure, Registration, and/or Certification Required:

Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA), and Advanced Cardiac Life Support certification (ACLS) issued by the American Heart Association (AHA)

Registered Nurse license issued by the state in which the team member practices.

Current MSN degree from an accredited Nursing school/program.

Experience Required:
Typically requires 2+ years of experience in clinical nursing.

Provides ongoing nursing care (assess, plan, intervention, evaluation) according to the age specific and developmental needs of the patient.

Assesses patient, family and/or significant others educational needs and provides teaching/counseling.

Anticipates and recognizes changes in a patient's status and need for care.

Determines priorities of patient care based on essential patient needs.

Integrates health promotion and preventive aspects of care.

Provides basic instruction for patient regarding routine procedures, treatments or disease processes.

Provides telephone triage to patient, family and/or significant others using nursing judgment according to protocol.

Assures quality of care through development, implementation and evaluation of professional nursing standards. Participates in quality assurance activities.

Participates in community education programs.

Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department’s policies and procedures. Age-specific information is developed further in the departmental job standards.

Excellent communication (written and verbal) and interpersonal skills; ability to develop rapport and maintain positive, professional relationships with a variety of patients, staff and physicians.

Demonstrated leadership skills including delegation, organization, prioritization and building team rapport.
Proficient computer skills including keyboarding, navigation within a windows operating system, use of electronic mail and electronic medical records systems.

Excellent interpersonal and customer service skills

Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required. Good driving record.

Physical Requirements and Working Conditions:
Must sit, stand, walk, lift, squat, bend, twist, crawl, kneel, climb, and reach above shoulders throughout the workday. Drive to and from patients place of service up to eight hours per day.

Must be able to:
lift up to 50 lbs from floor to waist.
lift up to 20 lbs over the head.
carry up to 40 lbs a reasonable distance.

Full range body motion including handling/lifting patients

Ability to drive up to 150 miles daily in urban and/or rural settings (company vehicle provided)

Ability to read and understand road maps

Ability to sit or stand for long periods of time

Must be able to:
push/pull with 30 lbs of force.
perform a sliding transfer of 150 lbs with a second person present.

Will be exposed to the following hazards on a frequent basis: mechanical, electrical, chemical, blood and body fluids. Will be required to wear protective clothing and equipment as needed.
Must have functional speech, vision, hearing, and touch with ability to use fine-hand manipulation skills.
Operates all equipment necessary to perform the job.
