Case Manager
At Oaks of Hebron we daily get to work with people who are one of the most vulnerable in our society with realizing their dream of living independently and interacting with their community. Oaks of Hebron services assist many people with developmental disabilities to be able to live, work and be in their community. We are currently looking for a Case Manager/ Community Support Facilitator.
As a Case Manager/Community Support Facilitator, you will manage a caseload of 10 to 12 clients, who live in their own home. With your desire to make a difference you will teach/support your clients the following:
Money management
Household chores and maintenance
Exercise and nutrition-healthy meal preparation
Recreational skills-social outings
Monitor medication and accompany clients to medical appointments
General social and emotional skills
Support clients with community integration and development of social skills through recreational activities
Develop and maintain positive relationships with third party agencies
Participate in client team meetings
Other life events
QUALIFICATIONS, REQUIREMENTS AND COMPENSATION
Education: Preferred but not required, bachelor's degree in social work, psychology, special education, or sociology.
Experience: This position requires a work background providing services to adults with developmental disabilities.
Reliable Transportation
A valid California Driver’s License, a working vehicle and legal minimum California Drivers Insurance to operate their vehicle and be insurable on the company policy is required.
Applicants: must pass a background check and pre employment physical and drug screen.

Salary: $20.00 - $22.00 per hour
