Social Services Director
Tierra Rose Care Center is hiring a Full-Time Social Services Director for our facility. As the Social Services Director you will assure that the residents' personal and emotional needs are addressed and met.
PRIMARY DUTIES AND RESPONSIBILITIES:
Assess residents' cognition, mood and behavior and collaborate with multi-disciplinary team to develop a comprehensive care plan for each resident. Arrange and lead necessary family care conferences to discuss care plan and/or discharge plan as indicated. Assist as needed with patient and family communications, as well as communication with staff and resource education. Encourage resident and family participation in care planning process. Ensure that all psychotropic medications have appropriate diagnoses and risk-benefit assessments (when indicated), are care planned according to state and federal regulations and have complimentary strategies and monitors in place. Participate in ongoing education regarding psychosocial issues, compliance, as well as resident rights. Effectively communicate information concerning a resident's condition with other departments and government agencies. Participate in the discharge planning, providing information to residents and families of programs available.
QUALIFCATIONS:
Bachelor’s or master’s degree in behavioral sciences; or associate degree with two years' experience in long-term care.
Two years' experience in long-term care required.
Must have the ability to read, write and understand English.
Highly motivated toward providing excellence in care to seniors. Belief in a positive, active and professional attitude with staff members. Team oriented. Must be in good health and able to carry out all necessary functions of the position.
Strong computer skills
Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
