AFC Student Nurse Practitioner - Full-Time
Priority Professional Care, located in Hyde Park, Mass. is looking for motivated, experienced, Adult Foster Care Student Nurse Practitioner for nurse case manager internship in the Boston area. Nursing case management services take place in members' homes.
Job Responsibilities:
Completing and signing the designated screening tool as required
Completing the MDS-HC and other nursing assessments and coordinating all other applicable clinical assessments.
Developing and reviewing on an ongoing basis each member's AFC plan of care.
Selecting, training, evaluating, and supervising AFC caregivers in conjunction with the care manager.
Reviewing the physician summary.
Monitoring the health status of all members to ensure that all needed AFC services are properly delivered.
Reporting changes in the health status of any member to the member's physician.
Monthly review AFC caregiver logs.
Conducting monthly on-site home visits with each member for Level I or more often as the member's condition warrants for Level II.
Completing a nursing progress note corresponding with each on-site visit or encounter, or more often as the member's condition warrants.
Submitting a semi-annual health-status report to the member's primary care physician.
Planning for and implementing discharges from the AFC program.
Providing ongoing training to AFC caregivers on health and aging.
Minimum Qualifications:
Current nurse practitioner student at a local institution
Knowledge, Skills and Abilities:
Ability to provide and/or basic nursing with supervision
Basic computer skills
Note:
Access to personal vehicle is required for home visits.
Pay rate is the reimbursement per each home visit completed, not per "hour."
